Below are PDF versions of our forms. We will accept printed copies of every form available, with the exception of the medical immunization form. We have provided a sample of the immunization form so you know what to ask your child's pediatrician for.

Please fill out each form as completely as possible. This will make the form acceptance process go much faster. Thanks!

Form Name Description
Continuing Student Registration This is for current students who will be continuing academic classes in the fall of 2010
New Student Enrollment Application This is for new students who would like to apply to Children's House
Treatment Release Form In case of emergency and we are unable to reach the student's emergency contacts
Permissions Form Grant permission for field trips, media photos, sunscreen, and/or the dental hygiene program
Statement of Health Health status for enrollment in a child care facility - must accompany the New Student Enrollment Application

Karen O'Hare
West Side Class Teacher and Spanish Teacher

Karen has been with the school since 1998. She has also taught at several other centers in the area. Karen teaches Spanish lessons to both classes daily in addition to teaching in the West Side class. She and her husband, Steve, have one daughter and two sons.
